PCV In -order to get this entitlement you will need to complete the following forms. D2 - Provisional Entitlement Form. D4 - Medical form. (Please call for doctors details). Information leaflet. You can obtain these forms from our office. Once your licence has come back you are ready to book a theory test (www.dsa.gov.uk). To book the test you will need to have your licence and credit card/debit card. Once you have completed the Theory Test you are ready for the Practical Training. Costs TRAIN SEVEN DAYS A WEEK. HOURLY-DAILY-WEEKLY LESSONS. TRAINING PROVIDED ON A ONE TO ONE BASIS. NO BINDING CONTRACTS. REFRESHER COURSES AVAILABLE SHORT NOTICE ASSESSMENTS ARRANGED. Effective from 1st July 2007.
